It looks like the dura mata has been separated from the skull, causing it to bulge/stretch, so i would guess at an epidural bleed, based on age, possible after fall and struck head.

hm... not sure, but sometimes in old people (especially in females) on some parts of the bones of the skull can thicken which causes a bulging deformity
