FACIAL TRAUMA RECONSTRUCTION!

A 56 year old man was admitted to the hospital as a tier III code trauma (GCS of 3). He was hit by an automobile while cycling and was pinned underneath the car resulting in fourth degree burn on the right side of his face as this portion was stuck under the muffler. He also suffered multiple rib fractures, pulmonary recontusion, nasal fracture, and bilateral fracture of iliac wings. For his facial trauma free flap coverage was performed within anterolateral thigh (ALT) flap. He almost completely lost the vision in his right eye, so he required two subsequent surgeries to optimise the contour of the right facial flap. Where did the ear come from as his seems to be gone in the injury pics? And how does he have perfect sideburn and eyebrow after reconstruction?

I believe he received a transplant. You can 3D print ear bones and have your ear surgically repaired.

What is the name of the scale they used to measure the trauma ? I haven’t heard before of tier III code trauma

Curious to know, too. The hospital I work at would have classified this as a level II trauma (surgeon has 12 hours to see patient) if not a level I (has 1 hour to consult patient). It depends on patient stability. Typically 'tier III' traumas are not announced overhead. And the trauma surgeon has 24 hours to see the patient.
