Extra ocular muscles are supplied by three cranial nerves namely: occulomotor (3rd), trochlear (4th) and abducent nerve (6th). The occulomotor supplies: medial rectus, inferior oblique, superior rectus and inferior rectus muscles. Superior oblique muscle gets its supply by trochlear nerve and lateral rectus muscle is supplied by abducents nerve. If there is 4th nerve palsy the eye will present as: eye up and out, the 6th nerve palsy is seen as: diplopia which increases on looking towards effected side.

SO4 LR 6 ... superior oblique by 4th nerve .. lateral rectus by 6th nerve
