Rhinoscleroma

Definition : --chronic specific granuloma caused by klebsiella rhinoscleromatis (gram negative bacilli) Pathology : --characterized by submucosal infilteration of lymphocytes, plasma cells, fibroblasts, mikulicz cells, Russel bodies Stages : --catarrhal stage -Atrophic stage -granulomatous stage -sclerotic stage Symptoms and signs: -foetid pus, crusting, epistaxis -anosmia, epiphora, nasal deformity -bilateral nasal obstruction -hard non ulcerative nodules filling nasal cavity with no tendency to involve bones Treatment : (medical) --streptomycin 1 GM IM daily -rifambicin 300 mg BID -other antibiotics according to culture sensitivity tests (surgical) -removal of localized nodules with plastic reconstruction
