A friend of mine complains of pain in the right arm since 6 months which sometimes radiates downwards to reach the forearm.The pain exacerbates on removing sweaters and raising the hand above the head. on consulting a doctor, he diagnosed it as a muskuloskeletal pain and prescribed a NSAID (Aceclofenac). The patient doesnot seem to be relieved from the treatment provided. what might me other differentials and the treatment?

PM and DM is associated to other autoimmune diseases like hypothyroidism. Thus you should check TSH, EMG and anti Jo-1 Ab for R/O these causes.

I think she has proximal weakness. Any history of thyroid disease? Rash on face or neck? FH of rheumatologic disease?
