A 60-year-old active male smoker had a swollen upper lip and lower cheek for the past 3 months, which had increased in size during the previous few weeks. No specific other symptoms were reported and his blood tests were normal. He had a smoking history of 300 packs per year for almost 40 years. CT scan of the head and neck showed he had a tender palpable mass measuring approximately 3·5 × 4·7cm on the upper lip and 5·6 × 3·2 cm on the lower cheek, both covered with normal skin. The mass on the cheek was subcutaneous. He neither had no history of injury or tumour.
