Mesiodens

A young 11 year old boy was referred to me with complain of missing tooth in left anterior maxilla for sometime. A/c to patients mother, his deciduous tooth was exfoliated when he was 7-8 years old and all permanent teeth erupted as usual apart from left central incisor. On clinical examination, missing central incisor was noticed alongwith a small buccal bulge at left central incisor region. OPG xray was done but on OPG xray, an extra tooth (mesiodens) was noticed along with the impacted central incisor. Now as we have to check which tooth is mesiodens and which one is central incisor, patient was advised to get his CBCT xray done. On CBCT xray, we concluded that the tooth which is placed buccally have its roots fully formed and its more naturally looking as compared to the palatally placed tooth which roots were not fully formed and differs in normal anatomy. So plan of Impacted Mesiodens extraction was made after consulting with an orthodontist, who suggest to not close extraction socket very tightly, So that central incisor will have a chance to erupt naturally, or else ortho treatment could be started if it doesn't erupt. Patient was told and counselled and extraction of mesiodens was done. This case was referred by a Junior Mesiodens is a supernumerary or an extra tooth that is present in the midline between two central incisors. It's prevalence varies between 0.09-2.05% in different studies.
