Testicular necrosis is a rare presentation of epididymitis in the emergency department. It is frequently associated with congenital anomalies that allow extreme mobility, rotating and achieving torsion of the vascular pedicle. Here we see the case study of a patient with progressive and intense pain in the right scrotum, lasting 12 hours prior to admission to the emergency department. Doppler ultrasound shows no vascularity in the corresponding testicle. A surgical exploration shows total necrosis of the same (visible in the image) due to testicular torsion.
