What is meant by Bilevel or biphasic ventilation mode of mechanical ventilation?
Airway pressure–release ventilation mode (APRV) of invasive mechanical ventilation is a newer modality that works on the principle of open lung approach. It is a continuous positive airway pressure (CPAP) that provides positive pressure which is timely regulated. Two different types of pressures are applied by APRV, the High-P that is coupled with Low-P. The High-P (CPAP) is given for a longer period (T high) of time and Low-P is given for a shorter period (T low). Longer duration of High-P helps in maintaining adequate lung volume and alveolar recruitment that is responsible for better oxygenation while the shorter duration of Low-P is responsible for expiration and removal of carbon dioxide. However this mode is associated with certain risks like volutrauma, increased work of breathing and bronchopleural fistula.
