What is your diagnosis ?

What is your diagnosis ?

A 4-year old boy presents blisters all over the body with an unknown cause at the moment. What diet might this child need if he is loosing a lot of fluid from his sores and experiencing hematuria? Please comment on foods that might help him as medications are doing no good. Thank you.

How old is this child? Could be bullous impetigo: flaccid, clear yellow fluid filled bullae that rupture and later become darker and more turbid; then they leave a thin brown crust. it has a Nikolsky sign posivity. A Gram stain and culture of pus is recommended to identify whether S.aureus and/or beta-hemolytic Streptococcus is the cause. The treatment may be initiated without these results. Unless cultures reveal only streptococci, the oral antibiotics should be effective for treatment of both S.aureus and streptococcal infection.

The lesions were continuous all throughout the year. Yes, he has recently developed hematuria.

He has been those lesions for a year? Continuously or intermittently? And know has developed hematuria?

Has he taken any drugs? Does it has pruritus? Nikolsky sign? Are mucous membranes involved?

Yes he has been given corticosteroids. There was no sign of improvement though.

It could be a skin disease like bullous pemphigoid or pemphigus foliaceus (fogo selvagem? Are you from South America?). Have you tried corticosteroids?

Long shot but what about Stevens-Johnson syndrome (SJS)? The only think that would turn me off from that is no blistering at the mouth

A short history please.

3 year old male child presenting blisters all over the body filled with clear fluid. There was no culture done but the medications he was put on for the lesions don't seem to help. He has had hematuria and positive Nikolsky sign. Currently on amoxicillin 250g and the lesions were drying up until we ran out of supply and it started getting worse again.

Any news? Did he have pharyngitis recently? We don't know if it is impetigo and if it's caused by Staphylococcus or Streptococcus. In case you think it's impetigo, you should begin treatment with dicloxacillin and cephalexin for 7 days p.o.

No sign of pharyngitis. He recently has been given 250g of amoxicillin and the lesions are slowly healing up but when they ran out of supply it came back on.
