I see angular cheilitis, glossy tongue (check for B12,Folate &iron deficiency anemias ),brown lesions could be Melanoma or BCC ?

I think Geographic tongue is more of a whitish map(this is more like hyperpigmentation) but this could be an atypical variant maybe, im not totally sure.

Ask about drug intake as linezolid, it makes brown or black tongue.. Also some iron preparations... investigate for peutz Jeghers syndrome

A black tongue is usually a harmless condition that can be caused by medications, smoking, poor oral hygiene, soft diet, or dry mouth. A black tongue is usually associated with elongated tongue papillae and thus, it is called "black hairy tongue" (lingua villosa nigra). The cause is thought to be a change in the normal bacteria in the mouth after antibiotic treatment or use of products that contain bismuth such as Pepto-Bismol.
