Serum Transaminase in Epstein Barr Virus Infectious Mononucleosis Normal range of serum transaminase is 30-50 IU/I. In more than 90% of the cases of EBV infection, LFTs are impaired. The level of serum aminotransferase and alkaline phosphatase are mildly elevated. The level of serum bilirubin is increased in approximately 40% cases. Increase in serum transaminase suggests viral hepatitis. Increase in serum alkaline phosphatase and gamma glutamyl transpeptidase (GGTP) levels do not suggest EBV infectious mononucleosis. Persistent elevation of serum transaminase over months with mononucleosis-like symptoms suggest cytomegalovirus infection.
