45 year old male, known case of diabetes melitus type 2, presented with c/c ulcers on perineum and scrotal skin since 3 weeks. Gradual onset, progressive and mild pain, associated with fever with rigors and pus discharge since last 4 days. He was diagnosed diabetic 4 years back and his diabetes is poorly controlled. No history of HTN, asthma, tuberculosis or allergy. He's diabetic (type 2), occasional smoker and alcoholic. On examination, tachycardia present and patient is febrile. Local examination reveals decreased pain sensation on the affected area and also have perepheral neuropathy. Multiple pus points seen on the ulcer.
