1 y.o boy presenting with tenderness all quadrants on his abdomen. History from his mother only anorexia in last 2 days and then become iritating. Physical findings suggested to a peritonitis on perforated appendix and on operative we found that perforated on base appendix and also the appendix was bigger than it should be. Perforation rates reported to be high as 82% in children younger than 5 years and nearly 100% of 1 year old. Diarrhea is most commonly seen with perforated appendix.
