Ascites....Most common cause:-liver cirrhosis. Other causes:-portal hyertension, CHF, kidney failure,etc.
Spleenomegaly, Hepatomegaly, perhaps Hemangioendothelomia
Its could be DD but all these conditions does not causes flank fullness i.e distension in flank region

Oesophageal varices,hepatic encephalopathy,
You forgot portal hypertension which can lead to Ascitis and it is a case of ascitis

Ascites but not because of insufficiency but due to portal hypertension
I think it is ascites it is happend because of aggragating fluide under the skin
