What is your diagnosis?

A 35 year old male was brought to the emergency department writhing in pain. He was severely hypotensive. There was no history of trauma, however, the patient's breath suggested excess alcohol intake. Upon removal of his shirt, this was observed. What are these signs and when do they occur?
