Q. A patient presented with hemoptysis and renal failure. Renal biopsy revealed crescentic glomerulonephritis and immunofluorescence revealed linear deposits of IgG. Which of the following is most likely the diagnosis? A. Acute post-infectious glomerulonephritis. B. Membranous nephropathy. C. Goodpasture syndrome D. Memranoproliferative glomerulonephritis.
Since there is heamptysis associated with renal failure, I guess "C"?
The correct answer is C. In Goodpasture syndrome patients develop anti-glomerular basement membrane antibodies (anti-GBM) resulting in crescentic glomerulonephritis, is characterized by linear deposits of IgG on immunofluorescence microscopy. Anti-GBM antibodies may cross-react with basement membranes of lung alveoli, resulting in simultaneous lung and kidney lesions.
basically, in good pasture syndrome antibodies are directed against globular non_collagenous component of collagen 4.Collagen 4 found in the basement membrane of Glomerulus. So patient present with protein urea ,hemturia etc. This is called Anti GBM glomerulonephritis.Same collagen 4 found in the BM of alveoli. So when antibodies attack this collagen then patient presents with Hemoptysis. So when Both glumeruli and alveoli are effected.this is called Good pasture syndrome.
