Retrosternal goitre

Etiology; ..retrosternal extension of the cervical thyroid goiter OR ..ectopic thyroid tissue in the thorax Presented by ;mediastinal syndrome (,compression manifestations) like dyspnea , dysphagia, hoarseness of voice, unequal pulse, congested neck veins, congested face, puffiness of eyelids, Pemberton sign ..ask the patient to raise his arms wirh some neck flexion for 3 minutes leading to congestion of face and respiratory distress If you percuss over the manubrium sterni , you can feel dullness Ttt..if it's malignant ;total thyroidectomy ..if simple goiter; subtotal thyroidectomy ..if toxic goiter; subtotal thyroidectomy but after preoperative preparation by B blockers

I think the treatment is from some old book because in practice we don’t do subtotal thyroidectomy due to high rate of recurrence Total thyroidectomy is preferred & then put the patient on Thyroxine life long
