Management of Acute Mastoiditis in Pediatric Age Group

Acute mastoiditis is one of the most common complication of otitis media in children. It is imperative to diagnose and treat acute mastoiditis immediately to prevent subsequent complications such as intracranial abscess, osteitis, facial nerve paralysis, and meningitis, among others. The management of acute mastoiditis is individualized i.e. treatment varies from patient to patient. The treatment plans may include parenteral antibiotics, myringotomy, sub-periosteal abscess incision and drainage, and mastoidectomy. Multidisciplinary approach, including a neurosurgical intervention, may help in devising a management plan. Also important is the possibility of suppression of classical signs and symptoms of complications of mastoiditis secondary to use of antibiotics, hence, appropriate diagnostic modalities should be employed to rule out such complications, in particular, the intracranial complications. The CRP levels along with CRP-to-albumin ratio are reliable indicators of intracranial complications, hence should be measured and monitored closely. If untreated, acute mastoiditis in children is associated with high mortality doe to its consequent complications. Timely diagnosis and intervention are necessary to improve patients’ outcomes.
