Huge inguinoscrotal swelling

45 years male patient with H/O large swelling in inguinoscrotal region for last 2 years, swelling was gradually increasing in size with occasional mild pain. On clinical examination cough impulse was positive, reducebility was absent, fluctuation was positive, trans illumination was positive in some part, getting above the swelling was not possible. Clinical diagnosis - B/L inguinal hernia with B/L hydrocele.
