A case of Aortic Aneurysm due to syphilis infection

This patient presented with a large pulsatile swelling of 4 months' duration on the left side of the chest. Initially, he noticed a small 4 × 4 cm swelling below the left clavicular region, which rapidly increased in size to about 10 × 10 cm in 4 months. He complained about breathlessness. No history of trauma. Local examination (right figure) showed a large swelling on the left side of the anterior chest wall. Chest roentgenogram (left) showing widening of the mediastinum with a radiopaque mass extending into the superior mediastinum occupying the left upper and mid-zone with a right tracheal shift. His blood serology was positive for syphilis and hepatitis B surface antigen. The patient was immediately admitted to the cardiothoracic surgery division for management.

Always amazes me that people can let something like this go on so long......it's large and pulsating and could have easily ruptured😕
