Had an elderly patient come into the preceptor office today. Prior to seeing her, she listed off a number of cardiac issues she has, the sum total being right sided congestive heart failure. After her check-up, the resident who had also worked up the patient before decided to show me her chest x-ray. She had a massive heart like the one above and even bigger. She had some noticeable pleural effusion in her CXR that I had heard in my auscultation of the back. This is perhaps the single most impressive thing I have seen in a CXR thus far.
