This isnt a typical injury you see everyday, the ball shaped head of the femur just popped out! The dislocation happened secondary to an extensive stage 4 full thickness decubitus ulcer, bedsore, or pressure ulcer of the buttocks. This is a skin ulcer caused by prolonged pressure against the skin resulting in completely or partially obstructed blood flow to the soft tissue (results from ischemic necrosis following continuous pressure on an area of skin that restricts microcirculation). Those lesions can ulcerate down to the bone and become infected, which further might lead to sepsis and death. In this case, the head of the femur came out of the cupâshaped acetabulum of the pelvis and through the open ulcer. Major skin breaks like the one seen above are treated with aggressive surgical debridement and antibiotics for the prophylaxis of possible infection. The patient also had an enormous abdominal incisional hernia, a type of hernia caused by an incompletely-healed surgical wound. A median incision in the abdomen is clearly seen, which was done for an exploratory surgery, leaving this patient at risk for hernia and a long hospital stay.
