The most common site for middle cerebral artery aneurysm MCA is its bi/trifurcation. This is a case of distal MCA aneurysm which is a rare. In such a case there is a high chance of mycotic aneurysm and you need to screen the patient for infections like endocarditis, HIV, viral hepatitis and syphilis.
