Cellulitis vs Erysipelas

#Celluities and #Erysipelas are due to infection in deep and superficial areas of dermis and subcutaneous tissue respectively.Cellulities is more common in lower limbs and arms which spread proximally.And also present in abdomen,peri anal and peri orbital areas.Erysipelas is more common in face and its sharply demarcated as its infected more superficially.These conditions present with tender inflamed areas with fever and malaise may associate with Blistering,Lymphangitis,Lympadenopathy and Abscess formation. Infectious agents are Beta haemolytic streptococcus and staphylococcus aureus.Diabetic patients and immunocompromized patients are more towards infections of gram negative bacteria and anaerobic bacterial infections Risk factors are Lymphoedema,leg ulcers,traumatic injuries, toe web impertigo In examination carefully look for any entry point for infections Skin swabs may be negative unless taken from broken skin Serological investigations to detect streptococcus are Antistreptolysin o titre and ASOT and can perform anti DNAase B titre Systemic antibiotics - fluxocilin or erythromycin 500mg 4 times a day are usually drugs of choice Toe web fungal infections should be treated to prevent recurrent infections
