Four years after endodontic therapy, the patient complained of pain and swelling associated with tooth #6. The initial impression was that apical surgery was indicated. however, further radiographs revealed the true cause of the endodontic failure. The initial endodontic access through the crown or caries damaged the coronal seal and recurrent decay followed.
