This is common feature in which two umbilical cords are getting entangled with each other. Fortunately, great fetal strips and 8/8 biophysical profiles were observed for their entire time in utero and babies were delivered healthy by C-section! Cord entanglement itself is rarely a problem but an associated tightening leading to an umbilical cord knot is a concern where arterial compromise can cause severe complications. These twins were admitted to the perinatal unit at 26 weeks gestation and delivered at 30 weeks. The mother had no significant medical history and other than multiple gestations, had an uncomplicated pregnancy. The condition can be closely monitored and assessed by Doppler if there's a concern for problematic blood flow due to tightening. Picture credit to aegRN
