Based on the presentation, Bacterial Endocarditis has been effectively ruled out, any form of embolism is usually emergent but this pt came in after a week,Bacterial meningitis is likely to have more Acute sxs like fever, +ve meningial irritation signs and acute onset of headache, Negative HIV effectively minimizes the chances of this being cryptococcal meningitis . Given a 40yrs hx of IVDA my working dx is a walled off infection (an abscess) on the left side of the brain is causing pressure on the brain tissue resulting in Right sided weakness.
