Long-term itching could have caused chronic prurigo with associated reactive perforating collagenosis. Now, it's hard to tell why the pruritis has happened, could be diabetes, malignancy, AIDS. Doesn't appear as much for pyoderma gangrenosum or necrolytic migratory erythema, and doubt malignant syphilis lesions would last this long. A biopsy of the lesions might have been helpful and history

It would appear to be atopic dermatitis but without more clarity that is merely a suggested candidate and the causes could be a multitude of illnesses.
