Is it recommended to continue ECT after the initial ECT session?
Continuation ECT (C-ECT) refers to the less frequent treatments given in the months after a rewarding index course of ECT while maintenance ECT (M-ECT) refers to less frequent treatments given several months after an index course ECT. C-ECT is given to prevent relapse while M-ECT is given to prevent a recurrence. Even though ECT is effective, the benefits are short-lived thus patients are given medications after ECT. If pharmacotherapy fails, C-ECT is recommended along with the medicines (combination of antidepressants and mood stabilizers) as it is the most successful combination. M-ECT is indicated when the patient has a history of ECT-responsive illness, patient preference or resistance or intolerance to medications. In the first few months post ECT, weekly C-ECT is conducted, which gets tapered to every other week and then eventually to once a month. The frequency of C-ECT or M-ECT depends on the patient’s condition and need. Since C-ECT is given less frequently than the index course, the side effects occur for a short duration and are not cumulative like in the index course.
