Large Granular Lymphocytic Leukemia

On the peripheral blood smear you can see large granular lymphocytes. Large granular lymphocytes will have small azurophilic granules in the cytoplasm. The flow cytometry histograms show loss of CD5 and CD62L in CD8 positive T cells with co-expression of CD57. The T cells will also express CD2, alpha-beta TCR. CD7 expression can be variable. The LGL cells are painted maroon on the histograms. By immunohistochemistry cytotoxic stains TIA1, granzyme B, and granzyme M will be positive on biopsies (like bone marrow biopsies).
