71 lady, 2 days post illeostomy and bowel resection. BP 68/39 mmHg HR 180/min Temp 38°C What's your diagnosis? And how You'll treat?
Not a fib, the rhythm is regular, with a P wave before every QRS. It looks like a 1st Deg AV block, with prolonged PR interval, rate of 180 bpm would be SVT. Also some ST depression in inferior and lateral leads (I, II, V4-V6) perform Right side 12 Lead, prepare for angioplasty/Cath lab. Also start a fluid bolus for pressure.
What if I say that patient responded to 12mg of Adenosine, switching back to sinus rhythm, HR 110/min (probably sepsis related) BP improved instantly to 130/80mmHg.
Then the rhythm is certainly SVT. Check cardiac enzymes, chest xray, lactate level for sepsis. And monitor patient for further issues. Might not need emergent cath.
