How Transthecal digital nerve block technique is performed?
Digital nerve block is a minor and easy to perform procedure that in done in cases of: • Different injuries (like fractures, lacerations, etc) • For different procedures (e.g. infection drainage and wound repair). The procedure involves injecting a regional anesthesia to anesthetize the area locally and specified nerves are blocked using this technique. TRANSTHECAL DIGITAL NERVE BLOCK • Hand is placed with the dorsal side facing the surface • Flexor tendon sheath is identified by palpating the palmar crease and agent is injected at 45 degrees distal to distal palmar crease. • There should not be any resistance while injecting the agent. If present, position the needle again by slowly pulling it backwards. A newer version is available where syringe is injected in metacarpal crease and is advanced until the bone is hit. • On hitting the bone slightly pull the needle backward and inject the anesthesia. • While injecting apply the pressure using the non-dominant hand before the injecting site so that the anesthetic agent injected flows distally.
