Unilateral Exophthalmos makes me think perhaps etiology is not hyperthyroidism(given its vast differential ), is this your patient(incase you could follow up and update us) ?

Unilateral exophthalmos may be due to tumor or edematous swelling because in hyperthyroidism there is bilateral exophthalmos.

I agree with you, we are on the same page i replied to u because you said in hyperthyroidism there is bilateral Exophthalmos, i meant to correct you that graves is the leading cause for both types Exophthalmos.

But we can not directly jump on thyroid deficiency on unilateral exopthalmos.thyroid shows its effect when more than 60 percent of it has been damaged.its just like u see a small mass on your body and diagnose it as cancer.
