Ddx: scurvey , phrnytoin ttt, gingivitis , leukemia, blood dyscrasias.

Acute leukemia related to hyperplasia of monocytes . Either Acute Monocytic L(>80% monoctes) or Acute mielomonocytic anemia. Can be drug induced as well- Phenytoin
