A 25 year old women with a 4 year history of extensive warts on both feet was presented to a plastic surgeon. This women also had undergone heart transplantation and this lesion was first developed, 7 years after the transplant. She has been receiving immuno-suppresive therapy with mycophenolate mofetil and tacrolimus since transplantation. Previous treatment for warts include use of topical chemicals like salicylic acid, carbon dioxide laser therapy. DIAGNOSIS: It is plantar warts. A physical examination revealed warty growth on plantar surface of metatarsal and heel region of both feet, with associated lower pedal edema. This infection is caused by human papillomavirus. TREATMENT: The patient underwent one aggressive debulking procedure. followed by treatment with topical cidofovir twice a day for a month. Over these 3 months the lesion gradually disappeared. She became able to bear weight without difficulty and her quality of life improved.
