Depends on the cause of pain and degree of renal damage. Considering it's diabetes, it may be a diabetic foot or peripheral arterial disease leading to ischemic pain so management will be planned accordingly

Thanks, it might be diabetic peripheral arthritis. If so then will a selective Cox-2 derivative eg: celebrex be the best choice of NSAID?
