A 54 year old Male, presents to the ward with a bout of hemoptysis. He complains of cough since 4 weeks with fever. He also complains of occasional night sweats. On investigation, his CD4 count was found to be 40. What is your likely diagnosis? 1. Mycobacterium tuberculosis 2. Klebshella pneumonie 3. Mycobacterium avium intracellulare 4. Streptococcus pneumonie
The correct answer is Mycobacterium Avium Intracellular Complex. Any patient with HIV and a CD4 count less than 50 will have MAC more likely. If his CD4 is between 200-50 then it can be M. Tb. The peculiarity about MAC is that it resembles M. Tb in clinical signs and symptoms.
