Hi doctors! Here’s a tough case for you. 19 Y/O female presenting with severe pain in the hips,knees and pelvis area. Constant pain worsening over time. Occasional flare ups where the entire body becomes immobile due to such intense pain. A number of months ago presented with what appeared to be cellulitis in the left leg with odd sudden onset and no visible cuts or entrance points for bacteria to cause cellulitis. Patient was a gymnast and has wear and tear in the knee joints. Onset of these flare ups tends to be near the menstruel cycle. Pain is not cramping or normal period pain but severe muscle and joint pain to the point of immobility as well as flu-like systems. Patient also has days where she can’t stand for more than 5-10 minutes at a time due to exhaustion and leg pain. This condition is worsening over time. Overall very healthy individual normally. Tested for lupus, rheumatoid arthritis, Anklosying spondylitis, and had full panel run with all results and x-rays appearing normal. This case has been brought to many many doctors with no results. Any ideas? What would you investigate for?
