if the patient comes from Latin America or has recently been there it may be cutaneous leshmaniasis

Yes consider infectious (Leishmaniasis etc) or lymphoprolif (cutaneous T-cell malignancies), but also ask if had any procedures done that could have lead to this (the practice of cupping)
