Pain is what the patient tell you as in "I feel pain in my abdomen" hence it's the symptom because it is what the patient actually verbalized to you. When you examine the patient and the patient winces, then that's the sign. A symptom is what the patient verbalizes to you but a sign is what you find on physical exam
