This is profound ascites and looking at the muscle wasting is consistent with protein-calorie malnutrition also known as Kwashiorkor’s disease

Was this child treated? And the tag says Leishmaniasis, so is VL the phasophys for the ascites rather than marasmic kwashiorker?

Protein based malnutrition aka kwashiorkor. The ascites is because of 3rd spacing because of low protein levels in the blood. You have to be very diligent when feeding because you can cause fluid and electrolyte imbalances in the process. A jejunal feeding tube with an elemental feeding would be best tolerated initially. Until we solve the social problems that caused this to begin with we are not really civilized.

Not necessarily, when it is due to prolonged malnutrition you have muscle wasting in addition to low oncotic pressure which redistributed the fluid into the third spaces causing the ascites.
