Exploratory laparotomy or EX LAP preformed on a 58 year old male who presented with an infected abdominal mesh from a previous abdominal hernia. Repair performed few years prior with a permanent PLASTIC type mesh (unknown when or where specifically). The mesh contracted and cut the bowel creating a leak of stool into the abdomen which infected the mesh. It healed temporarily, but festered in chronic infection status in the abdomen. The patient presented with progressive abdominal pain and fevers. Extraction of the infected plastic permanent mesh was performed. This is one of the main reasons why we might want to avoid using permanent mesh in an abdominal hernia repair. Preferred are biological meshes which get incorporated within less than a year. These don’t cause problems compared to the permanent plastic mesh type material. Biological mesh are made from human, pigs, or cows (cadaveric dermis).
