What the reason behind this "POPEYE SIGN"

Hints-An old man present with large buldge on his left arm which becomes more prominent during flexion.He developed this 2days back during lifting a heavy object and felt sudden pain.

Yes,here the cause is complete rupture of long head of biceps tendon (confirmed after MRI).It always does not show this "popeye sign" i.e. biceps may look normal even after tendon rupture.
