Severe case of pemphigus vulgaris!

When I first saw this patient I tought he had some kind of accident. After I read his medical file I was blown away: his diagnosis was pemphigus vulgaris. A skin biopsy was performed to establish this diagnosis (intraepithelial vesicle formation, acantholysis, and Tzanck cells). Unfortunatelly the patient died 1day later.

People who have moderate disease the skin require extraordinary care, similar to the care given to people with severe burns. The main treatment is corticosteroids (orally or intravenously). In severe cases you can give azathioprine, cyclophosphamide, methotrexate, mycophenolate mofetil, cyclosporine, rituximab orΒ  immune globulin given by vein. You can also try plasma exchange to filter antibodies from the blood. In this case the dr tried to use a surgical mesh to maintain the organs inside the abdomen (look closer to the belly). Also, the patient received antibiotics and plasmapheresis. For the blood and pus the dr used hemostatic sponges (but it didn't work).
