The WHO 2008 definition, defines accelerated phase as patients with CML who show one or more of the following features: 1) 10 to 19% blasts in the peripheral blood or bone marrow, 2) Peripheral blood basophils ≥ 20%, 3) Platelets <100,000/microL, unrelated to therapy, 4) Platelets >1,000,000/microL, unresponsive to therapy, 5) Progressive splenomegaly and increasing white cell count, unresponsive to therapy, 6) Cytogenetic evolution (defined as the development of chromosomal abnormalities in addition to the Philadelphia chromosome. Though 5% blasts are considered as CML-CP, AP is defined as blasts equal to or more than 10% blasts (but less than 20%), so 5–9% blasts are probably in CML-CP. Sokal criteria identifies ≥ 5% blasts in peripheral smear as accelerated phase (Ref. NCCN guidelines for CML).
