Hi guys..I’m a student from India.i have a doubt in pharmacology 1) why don’t we use direct acting cholinergics for treating myasthenia and other cases. 2) why don’t we use indirect acting drugs for glaucoma etc. 3) the presynaptic sympathetic ganglia also releases Ach so why wouldn’t it cause sympathetic symptoms during cholinergic crisis

Directly acting cholinergics have more of muscarinic action where as neostigmine has more of nicotinic action.

Same thing for the second question, M3 receptors are muscarinic so neostigmine has more of nicotinic action. So Less efficacious compared to pilocarpine.
