What's your diagnosis

*Patient with generalized pitting edema,clubbing of nails,scared lesions on body certain parts, *On X.ray she had cardiomegaly ,non tapering airways,pleural effusion,clustering alveoli and signet ring shaped apperance in lungs *She had hepatospleenomegaly *JVP was raised *on auscultations she had mixed type of crypt wheeze . #She belongs to Waziristan !can it be leshmeniasis?
