Pathogenesis of pyogenic osteomyelitis

The process begins as a focus of microabscess in a vascular loop in the marrow which expands to stimulate resorption of adjacent bony trabeculae. Simultaneously, there is beginning of reactive woven bone formation by the periosteum.
