Why is leishman skin test negative during the Kala azar disease condition ???? (in relation to pathogenesis)....

Leishmanin test is a delayed hypersensitivity reaction which is due to cellular immunity . In case of kala azar cellular immunity are impaired and not give a positive test.

In case of kala azar the parasite lie in visceral reticuloendothelial system and destroyed mononuclear cell of immune system ( macrophages ) which released IL 12 result in proliferation of T helper cell which further processed next step in cell mediated immune system. So due to loss of macrophages associated proliferation of CD 4+ cell impaire in CMI.
